Travel Physical Therapist - Home Health - $2,510 per week

Physical Therapist Home Health (PT HH) in Mount Angel, Oregon and surrounding areas. Start date: November 16, 2026. Duration: 13 weeks. This position involves providing outpatient home health therapy services within the community, focusing on patient-centered care to facilitate recovery and independence. The role requires delivering quality therapy to diverse patient populations in their homes, following established clinical protocols and maintaining comprehensive documentation.

Responsibilities

• Provide outpatient home health therapy services to patients in their homes.
• Deliver quality therapy to diverse patient populations.
• Follow established clinical protocols and organizational standards.
• Maintain comprehensive documentation of patient care.
• Communicate timely and adhere to travel compliance requirements.
• Submit Requested Time Off (RTO) at time of application.
• Manage own transportation within a maximum daily drive radius of approximately 60 miles.

Requirements

• Valid physical therapist license in the state of Oregon.
• BCLS certification.
• Minimum of 1 year of skilled home health experience.
• Proficiency in Homecare Homebase (HCHB) system.
• Active driver's license and personal transportation.
• License in-hand at time of submission.
• Open to first-time travelers.

Qualifications

Schedule
