Travel Physical Therapist Home Health - $2,432 per week

We’re currently looking for a PT I for a Home Health contract in Franklinton, NC. The position is a 13‑week travel assignment, 40 hours per week with 8‑hour shifts, serving patients in their homes and care facilities across Vance and Warren Counties.

Responsibilities

• Provide physical therapy services to patients in their homes and care facilities.
• Conduct start of care visits, follow‑ups, and other home health activities.
• Document patient assessments and treatment plans using electronic medical records such as HomeCare, HomeBase, Oasis, and Outlook.
• Travel up to 75 miles from the home clinic to serve Vance and Warren Counties.
• Rotate working a Saturday per month for social on‑call needs.

Requirements

• Current license in hand.
• Recent adult home health experience.
• CPR/BLS certification.
• Reliable transportation and agency‑required liability insurance.

Pay

Pay range: $2,327.40 – $2,443.77 per week (based on 8‑hour shifts and 40 hours per week).

Schedule

Schedule: 08:00 – 17:00, 40 hours per week, 8‑hour shifts. Includes rotating Saturday on‑call once per month.
