Responsibilities

• Provide physical therapy services in accordance with established protocols and standards.
• Assess patients' physical condition and develop treatment plans tailored to their needs.
• Document patient progress and communicate effectively with other healthcare providers.
• Ensure compliance with regulatory requirements and insurance company guidelines.

Requirements

• Active California Physical Therapist License.
• 1-2 years of experience in Home Health or Hospice settings.
• Proficiency with Home Care Home Base (HCHB) and PointCare.

• Strong communication skills to work effectively with patients, families, and interdisciplinary teams.
• Ability to manage time efficiently and prioritize tasks under pressure.

Benefits

• $55/hr Regular, $82.50/hr Overtime.
