About the role

We are seeking an experienced Physical Therapist (PT) for a Home Health opportunity based out of Urbandale, Iowa. Both travel and local candidates are welcome. This is a Monday–Friday position with no weekends and no call, offering 40 guaranteed weekly hours for travelers.

• Location: Urbandale, IA
• Setting: Home Health
• Schedule: Monday–Friday, 8-hour day shifts
• Guaranteed Hours: 40 hours/week for travelers
• No weekends, no on-call
• Orientation: Urbandale, IA
• Coverage: approximately 3–4 counties; visits scheduled geographically within 30–40 minutes of a centralized location

Responsibilities

• Perform PT evaluations and re-evaluations
• Complete PT discharges
• Provide skilled physical therapy services in the home setting
• Maintain approximately 6 productivity units per day (evaluations/re-evaluations count as ~1.5 units each; ~4 evaluations/re-evaluations per day may equal the expected 6 units)
• No Start of Care (SOC) assessments required—another PT handles SOCs

Requirements

• Active Physical Therapist license
• For travelers: previous professional PT experience required; no new-grad travelers
• Home Health experience highly preferred (clinical rotations during PT school may be considered)
• OASIS experience preferred, but not required
• Must hold a license from a PT Compact-eligible/participating state if not currently licensed in Iowa (traditional non-compact Iowa licensure candidates will not be considered due to potential delays)

Local PTs are encouraged to apply; additional flexibility regarding experience may be offered for local candidates due to potential long-term opportunities.
