Travel Physical Therapist - $3,247 per week

About the Role

Provide home health physical therapy services across the Silverdale, WA service area. The assignment covers two regions: North (Bainbridge Island, Kingston, Hansville, Poulsbo) and South (Bremerton, Port Orchard, Olalla). Minimum 30 home health units required; 32 hours guaranteed. HCHB (HomeCare HomeBase) and OASIS experience required.

Schedule

• Monday–Friday, 8:00 AM–5:00 PM
• One weekend every sixth week

Requirements

• Current Washington State Physical Therapist license
• Current CPR certification
• Valid driver’s license
• Reliable transportation and current auto insurance
• 1–2 years of experience as a Physical Therapist in an acute care or rehabilitation setting
• Bachelor of Science in Physical Therapy from an APTA‑approved program
• Previous home health/home care experience (preferred)
• HCHB (HomeCare HomeBase) EMR experience (preferred)

Responsibilities

• Provide physical therapy services to patients in their homes
• Evaluate patients and develop individualized treatment plans
• Deliver therapeutic interventions based on patient needs and physician orders
• Monitor patient progress and adjust treatment plans as appropriate
• Document assessments, treatments, progress, and outcomes accurately
• Collaborate with physicians, nurses, caregivers, and other healthcare team members
• Educate patients and caregivers on appropriate exercises, mobility, safety, and home programs
• Travel throughout assigned North or South service areas to provide patient care

Pay

Pay package is based on 8‑hour shifts and 40 hours per week (subject to confirmation) with a tax‑free stipend amount to be determined.
