About the role
LanceSoft is seeking a travel Physical Therapist for a 13-week assignment starting on August 10, 2026, in Portland, Oregon.
Responsibilities
- Provide physical therapy services to patients in a skilled nursing facility setting.
- Develop and implement individualized treatment plans based on patient assessment.
- Document patient progress and treatment outcomes accurately.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Adhere to all regulatory and organizational standards.
Requirements
- Both Oregon (OR) and Washington (WA) RN licenses are required.
- Pending licenses accepted, but the traveler must obtain both OR and WA licensure.
- Minimum 1 year of skilled hospice RN experience.
- BLS41 (3369656) certification required.
Qualifications
- Current CPR certification.
- Experience working in a skilled nursing facility setting preferred.
Skills
- Skilled in physical therapy techniques.
