Job Details

• Location: White River Junction, VT area; Lebanon, NH area; Bradford, VT area; Windsor, VT area
• Schedule: Days shift, 8 hours per day, a minimum of 40 hours per week

Responsibilities

• Screen, evaluate, re-evaluate, and treat patients
• Coordinate and communicate treatment activities with interdisciplinary team members and the patient
• Develop and carry out appropriate treatment programs
• Document evaluations and treatment goals
• Educate patients and families on treatments and procedures

Required Qualifications

• BS in Physical Therapy
• Must be a licensed Physical Therapist in Vermont and New Hampshire, or hold an unencumbered compact license
• One year of experience as a PT in a home healthcare setting
