Travel Physical Therapist - $2,875 per week

LanceSoft is seeking a travel Physical Therapist for a 13-week position starting August 2, 2026, in Portland, Oregon.

Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide therapeutic exercises, gait training, balance training, mobility improvement, and fall prevention education.
• Assess functional status and monitor patient progress toward rehabilitation goals.
• Complete accurate OASIS assessments and clinical documentation.
• Collaborate with physicians, nurses, and interdisciplinary team members to coordinate patient care.
• Document patient care using Epic Remote Client, Rover, and OASIS.

Requirements

• Active Physical Therapist (PT) license (Oregon or eligible; pending license accepted).
• Minimum 1 year of Skilled Home Health PT experience.
• OASIS experience (required).
• Current BLS (BCLS) certification.
• Valid driver's license, reliable personal vehicle, and auto insurance.
• Covid-19 primary vaccination series plus one booster (required).
• HbA1c (3344113) - not specified.
