About the role
The Physical Therapist Home Health (PT HH) position is located in Minneapolis, Minnesota. This role involves assessing clients' needs, developing personalized therapy plans, and performing therapeutic interventions in a home health setting. The position offers the opportunity to provide direct patient care while collaborating with interdisciplinary teams to support patient independence and recovery.
Responsibilities
- Implement therapy plans and maintain client confidentiality
- Notify physicians of changes in client status
- Document services provided
- Supervise therapy assistants in accordance with federal and state regulations
- Follow infection control standards and use appropriate precautions
Requirements
- Completion of an accredited physical therapy program
- Current BLS (Basic Life Support) certification
- State licensure and registration if applicable
- Minimum 1 year of clinical experience
- Ability to communicate effectively in English
Preferred Qualifications
- Prior experience in a home health setting (helpful but not required)
Schedule
- 8-hour days; open to 4-10s
- Minimum of 40 hours per week
- Float rotation may be required
