Travel Physical Therapist - $2,797 per week
Ardor Health Solutions is seeking an experienced Physical Therapist for a 13-week Home Health assignment in Delco, North Carolina. This role offers one-on-one patient care in a home health setting, working with adult and geriatric patients to improve mobility, strength, balance, safety, and independence as part of an interdisciplinary team.
About the Role
- 13-week travel contract in a home health setting
- 5x8-hour day schedule (40 hours per week)
- One-on-one patient care with adult and geriatric populations
- Independent, territory-based scheduling
- Collaborative interdisciplinary care team

Responsibilities
- Complete PT evaluations and reassessments in patients' homes
- Develop individualized plans of care
- Provide gait, balance, strengthening, transfer, and mobility training
- Treat orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex patients
- Provide fall-prevention and home-safety education
- Educate patients and caregivers on home exercise programs
- Coordinate care with nursing, OT, SLP, physicians, and other healthcare professionals
- Complete timely and accurate documentation using Homecare Homebase (HCHB)
Requirements
- Current North Carolina Physical Therapist license or ability to obtain prior to start
- Graduate of an accredited Physical Therapy program
- Current BLS certification
- Previous Home Health experience required
- OASIS experience required
- Homecare Homebase (HCHB) experience required
- Ability to independently manage a home health caseload and daily territory
- Strong clinical assessment, documentation, and time-management skills
- Valid driver's license and reliable transportation
- Comfortable traveling throughout the assigned service area
- Strong communication skills and ability to coordinate care with an interdisciplinary team
