About the role
LanceSoft is seeking a travel Physical Therapist for a 13-week assignment starting on August 24, 2026, in Rock Hill, South Carolina.
Responsibilities
- Provide physical therapy services to patients in a home health setting.
- Assess patient conditions using OASIS-E and chart accordingly in Homecare Homebase EMR.
- Manage IV, pleurx, wound vac, and wound care treatments.
- Teach disease management and provide geriatric/Adult chronic disease management.
- Document maintenance notes as needed.
Requirements
- A minimum of 1 year of experience as a Physical Therapist.
- BLS certification and state licensure required.
- A BA degree is preferred.
- Charting experience with Homecare Homebase is required.
- Experience with geriatric/Adult, chronic disease management, and maintenance notes.
Qualifications
- Driver's License and proof of insurance required.
- Experience with IV, pleurx, wound vac, and wound care treatments.
- Ability to travel up to 100 miles depending on case load.
- Experience with Homecare Homebase EMR.
Skills
- Charting experience with Homecare Homebase.
- OASIS-E assessment tool experience.
