Travel Physical Therapist - $2,755 per week
Job Summary
The position is for a Home Health Registered Nurse (RN) located in Newton, Massachusetts. The role involves assessing patient conditions, implementing care plans, providing ongoing education and support to patients and their families, and performing admission processes and case management responsibilities in accordance with organizational policies and standards of community health practice. Candidates are required to bring their license on the first day.

Schedule: Days, 8 hours per shift, minimum of 40 hours per week
Required Qualifications
1 year Certified Home Care experience
1 year Case Management Experience
Ability to travel to cases as assigned
Experience within a certified home health agency
