Wellspring Nurse Source is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a position in Pembroke, North Carolina.

Responsibilities

• Provide physical therapy services to patients in a skilled nursing facility setting.
• Develop and implement individualized treatment plans based on patient needs.
• Assess patient progress and adjust treatment plans as necessary.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Document patient care activities and progress in accordance with regulatory requirements.

Requirements

• Current licensure as a Physical Therapist in North Carolina.
• At least two years of recent relevant experience in a skilled nursing facility setting.
• Knowledge of Medicare and Medicaid regulations.
• Excellent communication and interpersonal skills.

Qualifications

• Master's degree in Physical Therapy from an accredited program.
• Passing score on the National Physical Therapy Examination (NPTE).
• Current CPR certification.

• Effective problem-solving abilities.
• Ability to work independently and as part of a team.
