Certification Details

• BLS certification (AHA or ARC only) required for onboarding.

Job Requirements

• FMOLHS Skills Checklist required within one year prior to start date; updated annually.
• FMOLHS Core Competencies must be current within a year prior to start with passing score of 80% or higher; updated annually.
• Proof of MMR immunization (2 immunizations, positive titer, or declination) required.
• Fit Test (N95 mask types) must be completed within 1 year prior to start; updated annually.
• Proof of Hepatitis B immunization (3 immunizations, positive titer, or declination) required.
• Proof of TB test (QuantiFERON Blood Test within one year prior or 2-Step TB Skin Test) required.
• Health Statement signed and dated within 1 year of start stating ability to work without restrictions required.
• Proof of Varicella immunization (2 immunizations, positive titer, or declination) required.
• Proof of Tdap vaccination within past 10 years or declination required.
• Annual TB Questionnaire required within 6 months of start.
• FMOLHS Criminal Background Check Results must be completed within 30 days prior to start.
• Negative 10 Panel Drug Screen within 30 days prior to start required.
• FMOLHS Competency Exam with passing score of 80% or higher within year of start required.
• FMOLHS Badge Photo required for onboarding.
• FMOLHS Contract Annual Education Packet required for onboarding.
• FMOLHS Eye Exam & Color Vision (Ishihara Color Test and Snellen Vision test) within 1 year prior to start required.
