Job Details: Job Title: General | Physical Therapist Location: Chapmanville, WV Setting: Home Health Pay Range: 2614.60 - 2745.33 Schedule: Discuss with recruiter. Contract Length: 13 weeks Candidate Type: Traveler Requirements: Current State License as a Physical Therapist; Proof of current CPR certification; Current driver's license. Must have reliable transportation and insurance; 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting; Previous home care experience Responsibilities: Perform clinical assessment to identify patient needs and appropriateness of patient care. Document, prepare, review, and complete a Plan of Care (POC), progress notes, phone orders, and other clinical record documentation. Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessments. Assist and instruct patients and families in transfer techniques, use of equipment, and therapeutic procedures. Collaborate with interdisciplinary team to develop and evaluate patient care plans.
