Job Description
Ardor Health Solutions is seeking a Physical Therapist for a 13-week Home Health assignment in Brunswick, Georgia. This opportunity is with CenterWell Home Health and offers the chance to provide one-on-one patient care throughout the Brunswick and surrounding coastal Georgia territory.
The Physical Therapist will evaluate patients, develop individualized treatment plans, provide skilled therapy services in the home setting, complete timely documentation, and collaborate with physicians, nurses, and interdisciplinary team members to achieve optimal patient outcomes. This assignment is ideal for therapists who enjoy autonomy, relationship-based patient care, and the flexibility of the home health setting.
Requirements
- Active Georgia Physical Therapist license (or ability to obtain)
- Minimum 1 year of recent Home Health experience preferred
- OASIS experience preferred
- Homecare Homebase (HCHB) experience preferred
- Current BLS certification
- Reliable transportation
- Ability to work independently and manage a field-based caseload
