Noor Inc. is seeking a travel Physical Therapist for a 13-week assignment in Albuquerque, New Mexico. This role offers an estimated gross weekly pay of $2710.40 for 40 hours per week, with 8-hour shifts.

Responsibilities

  • Provide physical therapy services within the legal scope of practice following all state licensure requirements.
  • Conduct initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications in accordance with Medicare and state standards.
  • Develop, implement, and update individualized plans of care aligned with physician orders and patient needs.
  • Deliver skilled interventions and maintenance therapy programs as permitted by state-defined scope of practice.
  • Directly supervise Physical Therapist Assistants (PTAs) and Home Health Aides in compliance with state practice acts.
  • Consult with physicians and interdisciplinary team members regarding treatment adjustments and patient progress.
  • Educate patients and caregivers on home exercise programs, mobility, and adaptive strategies to promote independence and safety.
  • Prepare and submit OASIS assessments, recertifications, transfers, resumptions of care, and discharges within agency-defined timeframes.
  • Apply ICD-10 coding accurately for compliant documentation.
  • Present in-service programs periodically to agency staff and contribute to staff education initiatives.
  • Participate in Quality Assessment and Performance Improvement (QAPI) activities.
  • Document clinical interventions, care coordination, and communication in a timely manner.
  • Participate in interdisciplinary meetings, case reviews, and continuing education opportunities.
  • Work independently with minimal daily oversight within a 45-mile radius of the Albuquerque area (including Tijeras, Rio Rancho, Bernalillo, Bosque Farms, Los Lunas, and Belen).
  • Maintain compliance with HIPAA and all privacy and confidentiality standards.
  • Demonstrate proficiency with organizational technology systems including communication, documentation, productivity, and data management platforms.

Requirements

  • Graduate of an accredited physical therapist curriculum.
  • Current Physical Therapist state licensure.
  • Two years of clinical experience preferred; home health experience a plus but not required.
  • Current CPR certification.
  • Reliable transportation, valid driver's license, and auto insurance required for home visits.
  • Basic computer proficiency and willingness to learn EMR systems and OASIS documentation.
  • Ability to perform essential functions of the role, including standing for extended periods, bending, reaching, and lifting up to 50 pounds occasionally, with or without reasonable accommodation.
