Wellspring Nurse Source is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a position in Eatontown, New Jersey.

Responsibilities

• Provide physical therapy services to patients in a skilled nursing facility setting.
• Develop and implement individualized treatment plans based on patient needs.
• Assess patient progress and modify treatment plans as necessary.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Document all patient interactions and treatments accurately and timely.

Requirements

• Current state licensure as a Physical Therapist.
• Experience working in a skilled nursing facility preferred.
• Knowledge of Medicare and Medicaid regulations.
• Excellent communication and interpersonal skills.

Qualifications

• Master's degree in Physical Therapy from an accredited program.
• Passing scores on the National Physical Therapy Examination (NPTE).
• Current CPR certification.

Benefits

Wellspring Nurse Source offers a competitive pay package including a tax-free stipend to be determined. The position includes 40 hours per week, 8-hour shifts, and a duration of 14 weeks starting July 13, 2026.

Pay

The pay package is based on 8 hour shifts and 40.0 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.
