About the role
The ideal candidate will provide high-quality, patient-centered therapy services in home settings. Responsibilities include conducting patient evaluations, developing individualized treatment plans, implementing therapy interventions, monitoring patient progress, documenting care, participating in case conferences, and collaborating with interdisciplinary teams.

- Document patient care and progress accurately in Homecare Homebase.
- Participate in case conferences and collaborate with interdisciplinary teams.
- Evaluate and adapt treatment plans based on patient needs and outcomes.
Requirements
- Active Physical Therapist license.
- Experience in home health and OASIS documentation is required.
- Homecare Homebase documentation experience preferred; candidate may be considered for reduced rate training for the first two weeks.

Pay
Pay package is based on 8 hour shifts and 40.0 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
