Travel Physical Therapist - $2,639 per week

About the role

We are seeking a dedicated Home Health Physical Therapist to provide exceptional care to patients in East Portland, OR, and surrounding areas. The therapist will be responsible for evaluating patients, developing treatment plans, and implementing therapy interventions in a home health setting.

Responsibilities

• Perform patient evaluations and develop personalized treatment plans.
• Implement therapy interventions and monitor patient progress.
• Document patient care and progress using electronic health records (Epic Remote Client and Rover).
• Educate patients and families on home exercise programs and therapy goals.
• Ensure compliance with healthcare regulations and provide high-quality care.

Requirements

• 1 year of skilled home health experience required.
• Current licenses to practice as a Physical Therapist in Oregon and Washington are mandatory.
• BCLS certification is required.
• Oasis experience is required.

Pay

The pay package is based on 8 hour shifts and 40.0 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.

Schedule

This facility cannot accommodate 4x10s or 3x12s. Schedule Day Shift: 5x8-Hour (08:00 - 16:30).
