The Physical Therapist (PT) Home Health (PT HH) candidate will provide patient evaluations and therapeutic services in a home health setting in Dover, Delaware. • Location: Dover, Delaware
• Shift Hours: Monday through Friday, 8:00 a.m. to 5:00 p.m.

• Certifications: Current CPR certification
• Licensing: Current, unencumbered license to practice physical therapy specific to Delaware
• Education: Successful completion of a physical therapy program approved by the American Physical Therapy Association or equivalent accrediting bodies
• Skills: Familiarity with general use and functions of the computer, including internet, email, file navigation, online HR services, and online training programs; EMR System: HCHB (Homecare Homebase); Experience in Home Health: Required, with experience in HCHB preferred; Supervision & Responsibilities: Provide direct physical therapy, monitor patient progress, and participate in discharge planning and final assessments; Supervise no more than 2 full-time equivalent PTAs; Mileage: Variable weekly travel to patients' homes within Kent County, Delaware
• Productivity Expectations: Minimum of 30 points per week
• Reporting To: The designated Director of Operations / Clinical Manager, with approval from Angela Bivins for weekly time review
• Contract Duration: 13 weeks

Pay Package

The pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.
