Responsibilities
- Perform comprehensive physical therapy evaluations and reassessments.
- Develop individualized treatment plans based on each patient’s functional goals.
- Treat orthopedic, neurological, post-surgical, and medically complex patients.
- Provide gait training, balance training, therapeutic exercise, transfer training, and functional mobility interventions.
- Collaborate with nursing staff, physicians, OT, SLP, and case management.
- Educate patients and caregivers on safety strategies and home exercise programs.
- Complete timely documentation and progress reporting.
Requirements
- Skilled Nursing Facility / Post-Acute Rehab experience preferred.
- SNF or post-acute experience preferred.
- Physical Therapist (PT) license required.
- BLS certification required.
