About the role
The healthcare provider is seeking a licensed Physical Therapist specializing in Home Health services to serve in the Hyannis, Massachusetts area. The role involves evaluating new patients, establishing treatment goals, administering diagnostic tests, and implementing therapeutic modalities to assist adults and pediatric patients in restoring function and preventing disability. The position emphasizes effective communication with medical and allied health personnel, documentation of patient progress, and patient/family education.
Responsibilities
- Evaluate new patients and set measurable treatment goals for functional outcomes
- Administer diagnostic muscle, nerve, joint, and functional ability tests
- Carry out appropriate treatment modalities
- Create documentation reports and communicate effectively with the healthcare team
- Educate patients and families
- Contribute to departmental quality improvement initiatives
- Serve as a clinical resource and provide supervision or training as needed
Requirements
- Bachelor's degree in Physical Therapy from an accredited Physical Therapy Program
- Licensed to practice in the Commonwealth of Massachusetts
- Current Basic Life Support (BLS) certification
- Ability to read, write, and communicate effectively in English
Preferred Qualifications
- Documentation of Specialist Certification
- Experience in home health clinical settings
Schedule
- Days shift, 8 hours per day, minimum of 40 hours per week
