Centra Healthcare Allied is seeking a travel Physical Therapist for a contract assignment in Pleasanton, California.
About the role
A great Skilled Nursing Facility (SNF) / Long Term Care (LTC) is looking for an experienced or newly graduated Physical Therapist to join their growing team. This is a contract position with a days schedule.
This role assumes responsibility and accountability for incorporating the vision, values, mission, and critical goals of the organization into job performance.
Responsibilities
- Develop a plan of care for each physical therapy patient.
- Provide skilled physical therapy services/interventions in accordance with physician orders.
- Assess patient needs, plan for, evaluate, and modify care to meet goals of physical therapy interventions.
- Collaborate with all disciplines to plan and evaluate team goals for each patient.
Requirements
- Must have graduated from an accredited school.
- Current state license as a Registered Physical Therapist or proactively in the process of the application for current state licensure.
Schedule
- Start Date: ASAP
- Duration: 13 weeks
- 40 hours per week
- Shift: 8 hours, days
