Responsibilities
- Assess referred patients requiring rehabilitation services, including evaluating medical condition, functional capabilities, limitations, and restrictions.
- Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under physician direction.
- Set realistic and achievable goals for patients, document and record patient conditions, and educate patients and families on appropriate physical therapy methods.
- Develop a plan of care for each physical therapy patient.
- Provide skilled physical therapy services and interventions in accordance with physician orders.
- Assess patient needs, plan for, evaluate, and modify care to meet goals of physical therapy interventions.
- Collaborate with all disciplines to plan and evaluate team goals for each patient.
Requirements
- Doctorate degree in Physical Therapy OR a Bachelor’s degree in Physical Therapy and have passed the NPTE.
- Graduated from an accredited school.
- Minimum one year’s experience as a Physical Therapist.
- Current state license as a Registered Physical Therapist (PT) or proactively in the process of obtaining licensure.
- Current CPR certification.
- Additional requirements may be determined by the client facility.
