Travel Physical Therapist - $2,526 per week

Ardor Health Solutions is seeking a Travel Physical Therapist for a 13-week Skilled Nursing Facility contract in Seaford, Delaware. This post-acute rehab opportunity offers the chance to work with adult and geriatric patients while helping them improve strength, mobility, balance, and overall functional independence. • Skilled Nursing Facility / Post-Acute Rehab
• 13-Week Travel Contract
• Full-Time Schedule (40 hours per week, 8-hour day shifts)
• Adult & Geriatric Patient Population
• Collaborative Rehab Team

Responsibilities

• Perform comprehensive physical therapy evaluations and reassessments.
• Develop individualized treatment plans based on patient goals and physician orders.
• Treat orthopedic, neurological, post-surgical, and medically complex patients.
• Provide gait training, balance training, therapeutic exercise, transfer training, and functional mobility interventions.
• Collaborate with nursing staff, physicians, OT, SLP, and case management.
• Educate patients and caregivers on safety strategies and home exercise programs.
• Complete timely documentation and progress reporting.

Requirements

• Delaware PT License Required
• BLS Required
• SNF or post-acute experience preferred
