Responsibilities
- Actively monitor and amend patient visit schedules to maximize productivity and meet patient needs.
- Perform comprehensive assessments, including psychosocial and risk factors.
- Develop and document patient-specific plans of care with defined goals.
- Provide skilled physical therapy treatments and document patient progress.
- Coordinate patient care with physicians, nurses, and other clinical staff.
- Supervise and direct the activities of PTAs, students, and aides.
- Complete necessary documentation for billing and insurance compliance.
- Participate in training programs for medical and community staff.
- Conduct various patient-related health assessments and procedures.
- Ensure timely communication regarding changes in patient care schedules.
- Coordinate safe patient discharges and transfers in compliance with regulations.
Requirements
- Valid Physical Therapist license.
- Experience with OASIS assessments and documentation.
- Knowledge of federal and state regulations related to physical therapy.
