Travel Physical Therapist - $2,480 per week

About the role

Wellspring Nurse Source is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 14-week assignment starting October 5, 2026, in Laconia, New Hampshire.

Responsibilities

• Provide physical therapy services to patients in a skilled nursing facility setting.
• Develop and implement individualized treatment plans based on patient needs.
• Assess patient progress and adjust treatment plans accordingly.
• Collaborate with interdisciplinary teams to ensure comprehensive care.
• Document all treatments and patient progress in accordance with regulatory requirements.

Requirements

• Current licensure as a Physical Therapist in the state of New Hampshire.
• At least one year of experience in a skilled nursing facility setting preferred.
• Knowledge of Medicare and Medicaid regulations.
• Excellent communication and interpersonal skills.

Qualifications

• Master's degree in Physical Therapy from an accredited program.
• Passing scores on the National Physical Therapy Examination (NPTE).
• Current CPR certification.
