Travel Physical Therapist - $2,473 per week
Job Description & Requirements
Provide physical therapy services across assigned county territories including Area #1 (Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City) and Area #2 (Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont).
Additional travel outside of assigned areas may be required.
Weekend coverage may be required.
A guarantee option requires a minimum of 27 points per week with a 36-hour guarantee.
The guarantee does not apply for the first and last week of assignment, weeks with holidays, or weeks with approved time off.
A non-guarantee option allows patient refusal with no guaranteed hours and payment made on a per visit basis.
- Visit types include OASIS ReCertification, OASIS RoC, Routine, Eval, Discharge, Eval SoC, Eval RoC, Eval ReCert, Eval F/up, Re-Eval, Eval Discharge, OASIS Discharge, and OASIS F/up.

Pay Package
The pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.
