About the role

The Physical Therapist will evaluate all assigned patients and develop, implement, and modify treatment plans in accordance with the patient’s needs and physician direction. This role involves delegating and directing all discipline-specific therapy services for assigned patients and ensuring the design of functional treatment programs that facilitate effective community reintegration.

Responsibilities

  • Completes comprehensive evaluations of assigned patients and utilizes clinical reasoning to develop treatment plans and goals focused on progression towards discharge.
  • Collaborates with an interdisciplinary team to address discharge barriers, ensuring timely equipment trials and completing patient and family training.
  • Manages individual, concurrent, and group treatment sessions based on patient needs effectively and efficiently.
  • Documents medical records in a clear, concise, accurate, and timely manner, following state and federal guidelines.
  • Participates in student programs, fostering a positive learning environment with proper supervision and guidance.
  • Engages actively in professional development and advocacy efforts.
  • Maintains ethical standards and compliance with discipline-specific State Practice Acts and continuing education requirements.
  • Demonstrates awareness of organizational performance improvement initiatives and contributes feedback towards enhanced service quality.

Requirements

  • BLS Certification required prior to first shift.
  • Must hold a valid and current State license as a Physical Therapist.
  • A minimum of 2 years of experience required, with previous LTACH experience preferred.

Schedule

This position follows a Day 5x8.0 shift schedule.
