Zack Group is seeking a travel Physical Therapist for a position in Avondale, Arizona.

Responsibilities

• Provide physical therapy services to patients in need
• Develop and implement individualized treatment plans
• Assess patient progress and modify treatment plans accordingly
• Document patient care activities and progress in accordance with regulatory requirements

Requirements

• Current state or equivalent compact license
• At least 2 years of recent Physical Therapy - PT experience
• Active or pending State License as an Physical Therapy - PT
• Current BLS and/or Specialty Certifications for Physical Therapy - PT
• 2 current leadership references, 1 current co-worker

Qualifications

Must be able to pass background/urine drug screening.

Pay

Compensation and Benefits: Highly competitive pay rates. Pay package is based on 8 hour shifts and 40.0 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
