Evaluate patients, develop individualized treatment plans, and collaborate with the interdisciplinary team to maximize functional outcomes and prepare patients for a safe discharge home or to the next level of care. Responsibilities Perform comprehensive physical therapy evaluations Develop and implement individualized treatment plans Provide individual, concurrent, and group therapy sessions Collaborate with the interdisciplinary team to remove barriers to discharge Educate patients and families on mobility, safety, and home exercise programs Coordinate equipment recommendations and discharge planning Maintain accurate and timely documentation Participate in quality improvement and professional development initiatives Supervise students as assigned Required Qualifications Active Physical Therapist license in the state of Florida BLS Certification prior to first shift Minimum of 2 years of Physical Therapist experience Previous Inpatient Rehabilitation experience Preferred Qualifications Previous travel assignment experience Previous Select Medical assignment preferred Facility Highlights Select Medical Inpatient Rehabilitation Hospital Located in Lake Worth, Florida Team-oriented rehabilitation environment focused on helping patients regain independence following illness, injury, or surgery Additional Information 13-week contract Monday–Friday schedule Day shift (8:00 AM – 4:30 PM) Minimum of 2 weekend shifts per month Inpatient rehabilitation setting Start date: August 31Treva Workforce Job ID #807638.
