- Home Health Physical Therapist position with CenterWell Home Health
- Adult and geriatric patient population
- 13-week travel contract with an estimated weekly pay of $2,421
- 40 hours per week, 8-hour day shifts

Responsibilities
- Perform comprehensive patient evaluations and develop individualized Plans of Care (POCs)
- Provide treatment for neurological, neuromuscular, and orthopedic conditions in the home setting
- Complete all required documentation, including evaluations, progress notes, physician orders, and clinical records
- Collaborate with physicians, nurses, caregivers, and the interdisciplinary care team
- Provide patient and family education on treatment plans, home exercise programs, safety, and equipment use
- Assess and recommend durable medical equipment (e.g., walkers, wheelchairs, canes, braces, prosthetics)
- Instruct patients and caregivers on safe transfers, mobility techniques, and functional activities
- Participate in infection control and patient safety initiatives
- Travel locally throughout the assigned territory
Requirements
- Current Physical Therapist license
- Current CPR certification
- Valid driver’s license, reliable transportation, and proof of insurance
- 1–2 years of Physical Therapy experience in an acute care or rehabilitation setting
- Strong communication, documentation, and clinical assessment skills
- Ability to perform physical demands, including lifting, bending, pushing, pulling, prolonged standing, and walking
- Home Health experience preferred
- OASIS experience preferred
