Responsibilities
- Evaluate and treat participants' mobility and motor function for functional performance.
- Provide goal-directed, activity-oriented treatment to promote optimum independence in self-care, functional mobility, and daily life tasks.
- Ensure continuation of progress through effective education of the participant, family, or support system, and provide adaptive methods and equipment as appropriate.
- Participate in family/participant care-plan meetings, the interdisciplinary team (IDT), and committees as determined by the Director of Therapy.
- Plan, manage, and coordinate participant evaluations and treatments based on age-specific needs.
- Evaluate the whole participant — incorporating medical-record data to assess functional status, physical disability, movement dysfunction, pain, community mobility, and rehabilitation potential.
- Correlate evaluation data to determine the IDT plan of care, directed toward short- and long-term goals.
- Administer skilled, 1:1 treatment focused on posture, locomotion, strength, endurance, cardiopulmonary function, coordination, joint mobility, flexibility, pain, and healing/repair.
- Determine appropriateness of each participant's response to treatment and consult with the physician and team on care-plan modifications.
- Analyze tasks and adapt the environment, tools, materials, and treatments to participant needs and socio-cultural context.
- Participate in IDT conferences and family meetings as a team player ensuring continuity of care.
- Schedule participants according to caseload and demand, coordinating with other services.
- Delegate treatment to support personnel per their expertise while retaining responsibility for the participant and treatment program at all times.
- Follow all documentation and form-completion policies and procedures.
Requirements
- Excellent verbal, written, interpersonal, and customer-service skills.
- Excellent organizational skills and attention to detail.
- A caring attitude toward participants and their families.
- Strong time-management, analytical, and problem-solving skills with a proven ability to meet deadlines.
- Ability to function well in a fast-paced and at times stressful environment.
- Proficient with Microsoft Office Suite or related software.
Qualifications
- Active, unrestricted state Physical Therapist license.
- CPR/BLS certification.
- High school diploma or equivalent.
- Experience with geriatric, frail, or medically complex populations preferred.
Skills
- Constant standing, balance, grasping, and full-body range of motion.
- Occasional walking (up to ~0.25 mile).
- Sustained bending/squatting/kneeling.
- Reaching overhead.
- Lifting up to 100 lbs floor-to-waist.
- Push/pull up to 100 lbs of force.
- Exposure to blood/body fluids, communicable diseases, and other clinical environmental conditions.
