Travel Physical Therapist - $2,413 per week

About the role

A great SNF / LTC is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contract assignment.

Responsibilities

• Assess referred patients for rehabilitation services, including assessing medical condition, functional capabilities, limitations, and restrictions.
• Establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for patients, document and record patient's condition, and educate patients and families in appropriate physical therapy methods.

Requirements

• Graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.

Pay

Pay package is based on 8.0 hour shifts and 40.0 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.
