Travel Physical Therapist - $2,406 per week
About the role
LanceSoft is seeking a travel Physical Therapist for a 12-week assignment in Mansfield Center, Connecticut.
Responsibilities
- Perform comprehensive physical therapy evaluations in the home setting.
- Develop, implement, and modify individualized treatment plans based on patient needs.
- Provide skilled therapeutic interventions to improve strength, mobility, balance, and functional independence.
- Educate patients, families, and caregivers on treatment plans, home exercise programs, and injury prevention.
- Collaborate with physicians, nurses, case managers, and the interdisciplinary healthcare team.
- Maintain accurate, timely, and compliant clinical documentation.
- Travel throughout the assigned service territory to provide patient care.
Requirements
- Active Connecticut Physical Therapist (PT) License (required prior to start date).
- Minimum 1 year of recent Home Health Physical Therapy experience.
- Valid driver's license.
- Reliable transportation.
- Current automobile insurance.
- Acceptable Motor Vehicle Report (MVR).
