Wellspring Nurse Source is seeking a travel Physical Therapist for a 13‑week assignment in Bloomfield, Iowa. The role involves providing skilled physical therapy services in compliance with state and federal regulations, evaluating patients, developing individualized treatment plans, delivering direct therapy, and supervising PTAs and rehabilitation technicians.

Responsibilities

• Screen, evaluate, and treat patients as necessary.
• Develop and update treatment and discharge plans of care.
• Document services accurately and within required timelines.
• Supervise Physical Therapy Assistants (PTAs) and Rehabilitation Technicians as per practice acts.
• Collaborate with interdisciplinary care teams and family members.
• Maintain compliance with safety, infection control, and regulatory standards.

Requirements

• Graduation from an APTA‑accredited Physical Therapy program (DPT, MSPT, or BSPT).
• Active state Physical Therapy license or eligibility to obtain one.

Skills
