About the role
The Physical Therapist will provide specialized physical therapy services within the home health and hospice environment, delivering comprehensive, patient-centered care while ensuring accuracy, safety, and compliance with all regulatory standards.
Responsibilities
- Perform comprehensive physical therapy assessments and develop individualized, evidence-based treatment plans.
- Supervise and coordinate care with Physical Therapist Assistants (PTAs).
- Provide therapeutic interventions focused on mobility, strength, pain reduction, and functional independence.
- Collaborate with interdisciplinary team members to coordinate patient-centered care and discharge planning.
- Educate patients and caregivers on safe mobility techniques, equipment use, and home-exercise programs.
Requirements
- Licensed Physical Therapist in North Dakota.
- Current driver's license required and a car available to travel to patients' homes.
- Strongly preferred: 2 years previous experience as a Physical Therapist.
