Travel Physical Therapist - $2,133 per week
About the role
Provide home health physical therapy services, treating patients in various settings such as homes, assisted livings, and senior apartments. Responsible for assessments, planning, and implementing individualized treatment plans.
Responsibilities
- Treat 3 to 5 patients per day depending on visit type.
- Complete 2 Start of Care visits per day, or 3-4 evaluations/other visits.
- Supervise up to 2 different PTA/COTAs.
- Manage documentation, travel, and case management within shift hours.
- Participate in meetings twice a month for timesheet approvals.
Requirements
- Active BLS (AHA) and CA PT License.
- Home Health experience including OASIS required.
- 2 years experience in a healthcare setting required.
- Current driver's license and proof of automobile insurance.
- Good oral and written communication skills; good organizational skills.
- EMR HomeCare Home Base (HCHB) experience preferred.
Qualifications

Benefits
