About the role
CoreMedical Group is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 13-week assignment starting August 1, 2026.
Responsibilities
- Provide physical therapy services to patients in SNFs
- Develop and implement individualized treatment plans
- Evaluate patient progress and adjust treatment plans accordingly
- Document patient care activities and progress in medical records
- Collaborate with interdisciplinary teams to ensure comprehensive patient care
Requirements
- Current licensure as a Physical Therapist in CT
- At least 1 year of relevant clinical experience
- Passing scores on the NCLEX-PN or RN
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
Qualifications
- Master’s degree in Physical Therapy
- ACSM-CPT certification preferred

Pay
Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
Schedule
Days schedule, 40 hours per week, 8 hours per shift.
