Responsibilities
- Provide skilled physical therapy services to patients in their homes.
- Complete comprehensive evaluations and develop individualized plans of care.
- Help patients improve strength, balance, mobility, safety, and functional independence.
- Provide gait training, transfer training, therapeutic exercise, and fall‑prevention education.
- Educate patients and caregivers on home exercise programs and safe mobility.
- Collaborate with physicians, nurses, and other members of the Home Health care team.
- Complete accurate and timely clinical documentation.
Requirements
- Active New Hampshire Physical Therapist license or eligibility to obtain.
- Previous Home Health experience preferred.
- Current BLS certification.
- Strong time‑management, communication, and independent clinical decision‑making skills.
- Ability to travel between patient homes as required by the assignment.
