Job Summary

The organization is seeking a Physical Therapist to evaluate assigned outpatients and develop, implement, and modify individualized treatment plans based on patient needs, goals, and physician direction. The role includes maintaining accurate and timely clinical documentation, supporting student education as assigned, and contributing to discipline-specific program delivery and performance improvement efforts.

Job Details

  • Schedule: Days shift; 8 hours per shift, with a minimum of 40 hours per week.

Required Qualifications

  • Education: Degree in Physical Therapy from an accredited school
  • Clinical Experience: 1+ year of outpatient clinical experience
  • Licensure: Active professional licensure in good standing with completion of all state-mandated continuing education requirements

Additional Information

  • Responsibilities:
    • Completes comprehensive evaluations and uses clinical reasoning to develop treatment plans and goals focused on progressing patients toward discharge within an established length of stay.
    • Ensures care is provided under signed, valid physician orders and within insurance approval, as indicated and necessary for clinical care.
    • Completes medical record documentation, including goal writing, functional quality indicator assessment, letters of medical necessity, and charge entry using approved formats in compliance with state, federal, and intermediary guidelines, as well as clinic policies and procedures.
    • Participates in student program activities as assigned, including providing appropriate supervision and contributing to student learning objectives and evaluations.
    • Develops and maintains an individualized professional development plan, pursues ongoing professional growth through continuing education and self-study, and applies acquired knowledge to support organizational objectives.
    • Adheres to the discipline-specific State Practice Act and other regulatory agencies and performs in an ethical manner consistent with the discipline-specific professional code of conduct.
    • Supports organizational performance improvement efforts by following action plans and offering feedback through staff meetings or focus groups as appropriate.
    • Maintains compliance with confidentiality expectations regarding patient and organizational information and upholds professional standards and codes of conduct.

Pay Package

The pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.
