About the role
LRS Healthcare - Allied is seeking a travel Physical Therapist for a 13-week assignment starting on August 31, 2026, in Crown Point, Indiana.
Responsibilities
- Care for patients in a skilled nursing facility setting
- Develop and implement individualized treatment plans
- Assess patient needs and progress regularly
- Document patient care activities accurately
- Collaborate with interdisciplinary teams
Requirements
- Current license to practice physical therapy in the state of Indiana
- Experience in a skilled nursing facility setting preferred
- Pass a background check
Qualifications
- Master’s degree in Physical Therapy from an accredited program
- ACLS certification
- Knowledge of Medicare/Medicaid regulations

Pay
The pay package is based on 12-hour shifts and 36 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.
Schedule
Shifts will be 12 hours per day, 7 days a week.
