About the role
Provide rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF) as a Physical Therapist.
Responsibilities
- Perform evaluations, reassessments, treatments, and other clinical services.
- Develop and implement individualized treatment plans in collaboration with supervising therapist, physician, and team.
- Implement evidence-based therapeutic interventions.
- Educate residents, families, and caregivers on treatment plans and progress.
- Complete accurate documentation according to facility, state, federal, and payer requirements.
- Supervise therapy assistants when applicable.
Requirements
- Minimum two (2) years of experience in the SNF/ICF setting preferred.
- Current Basic Life Support (BLS) certification.
- Current State of Hawaii license in good standing appropriate to the discipline.
Schedule
- 13-week contract starting 09/14/2026.
- 40 hours per week, 10-hour shifts (07:00–17:00).
- Variable 7-day departmental coverage with a 5-day work week (flexible scheduling).
Pay
Pay package is based on 10-hour shifts and 40 hours per week, including a tax-free stipend (amount to be determined).
