Travel Physical Therapist - $1,909 per week

About the role

The Rehabilitation Therapist will provide comprehensive rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF).

Responsibilities

• Conduct evaluations, reassessments, treatments, and other clinical services in accordance with licensure and scope of practice.
• Develop, implement, and carry out individualized treatment plans in collaboration with supervising therapist, physician, and interdisciplinary teams.
• Implement evidence-based therapeutic interventions to maximize function, independence, communication, mobility, swallowing, and quality of life.
• Educate residents, families, and caregivers regarding treatment plans and patient progress.
• Complete accurate, timely, and compliant documentation of patient progress, treatment changes, and discharges per facility and regulatory requirements.
• Supervise therapy assistants as applicable to the role.
• Be available to work during Hawaii State and Federal Holidays as necessary based on facility operations.

Requirements

• Minimum of two (2) years of experience in a Skilled Nursing Facility (SNF) or Intermediate Care Facility (ICF) setting preferred.
• Current Basic Life Support (BLS) certification.
• Current State of Hawaii license in good standing appropriate to the discipline is required by start date.

Qualifications

• Graduation from an accredited Physical Therapy program.
• Current licensure as a Physical Therapist in the State of Hawaii.

Pay

Pay package is based on 12 hour shifts and 36.0 hours per week (subject to confirmation) with tax-free stipend amount to be determined.

Schedule

Variable schedule - 7 days per week departmental coverage with a generally scheduled 5-day work week. Day shift, including possible weekend days.
