Responsibilities

• Evaluate patients and develop individualized treatment plans
• Provide treatment focused on mobility, function and patient goals
• Modify plans of care based on patient progress
• Educate patients and families on treatment and home programs
• Complete accurate and timely clinical documentation
• Carefully coordinate care with physicians and the clinical team
• Supervise/delegate therapy services as appropriate
• Follow patient safety, infection-control and regulatory standards
• Maintain active PT licensure and required continuing education

Requirements

• Current physical therapist license in good standing
• Experience in outpatient physical therapy preferred
• Knowledge of patient safety and infection control protocols
• Ability to work flexible hours including evenings and weekends
