Job Details
Schedule: Days, 8 shift hours, with a minimum of 40 hours per week
Required Qualifications
- Active Occupational Therapist (OT) license
- Resume
- Skills checklist within 1 year
- 01 reference at time of submission
- Adherence to NBCOT and state-required practice acts and code of conduct/ethics during professional practice
- Able to complete required documentation and maintain patient records according to policy
- Provides screening, evaluation, and treatment planning for patients with cognitive and/or ADL physical functioning disorders
- Uses standardized assessments and evidence-based practice to support clinical interventions
- Delivers rehabilitative treatment interventions per regulatory and clinical practice requirements, including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and the use of supportive and assistive devices
- Affirms patient progress, adjusts treatments accordingly, and evaluates treatment outcomes for effectiveness
- Communicates with the healthcare team regarding patient progress, barriers, and treatment plans
- Completes comprehensive discharge summaries of services provided in accordance with regulatory and clinical requirements
- Documents all regulatory and clinically required information in the electronic documentation system and accurately records treatment minutes for services provided
- Participates in required meetings, including patient care conferences, utilization review meetings, and rehabilitation meetings
- Participates in facility and organization required trainings, in-services, and conferences
- Obtains necessary durable medical equipment (DME) or supplies per regulatory and clinical practice requirements
- Supervises and supports occupational therapist assistants, rehabilitation aides, and/or students
