Responsibilities

  • Perform clinical assessment to identify patient needs and determine appropriateness for occupational therapy.
  • Document, plan, implement, and revise a Plan of Care (POC); maintain progress notes and other clinical records.
  • Deliver direct therapeutic interventions addressing ROM, strength, coordination, adaptive equipment use, and environmental modifications to support ADL performance; fabricate and maintain adaptive equipment and hand splints as needed.
  • Evaluate patient occupational performance across self-care, work, leisure, and rest; assess sensory-motor, psycho-social, and cognitive-perceptual components; consider environmental and cultural factors.
  • Apply infection control and universal precautions; coordinate care with the interdisciplinary team; consult with physicians regarding patient status.
  • Communicate relevant patient information to physicians and caregivers while protecting confidentiality; maintain accurate records and assist with third-party payer education as needed.
  • Troubleshoot and perform basic maintenance and setup for OT equipment; participate in related duties as assigned.

Requirements

  • Current state license as an Occupational Therapist (active)
  • CPR certification (current)
  • Valid driver’s license; reliable transportation and auto insurance
  • Minimum of two years’ experience as an Occupational Therapist in an acute care or rehabilitation setting
  • Education: Bachelor of Science in Occupational Therapy from an accredited program
  • Ability to work in a home care environment and to travel locally as needed
  • Flexible schedule capability and willingness to travel locally
