Pay

$52.00 to $53.00 per hour* (pay based on experience, expertise, credentialing, and education). Mileage reimbursement: $0.51 per mile.

Duties

• Perform comprehensive occupational therapy evaluations and assessments in patient homes to identify functional limitations and establish treatment goals.
• Provide skilled occupational therapy interventions focused on improving independence with activities of daily living (ADLs), mobility, safety, and overall quality of life.
• Complete patient recertifications, reassessments, and discharge evaluations while ensuring compliance with agency, regulatory, and payer requirements.
• Document all patient encounters accurately and timely within Homecare Homebase (HCHB) and the electronic medical record system.
• Collaborate with physicians, nurses, therapists, caregivers, and interdisciplinary team members to develop and implement individualized care plans.

Requirements

• Graduate of an accredited Occupational Therapy program.
• Active Washington State Occupational Therapist license.
• Current BLS certification (preferred).
• Minimum 1 year of Home Health experience preferred.
• Proficiency with Home Health Occupational Therapy, HCHB, patient assessments, recertifications & discharge planning, functional mobility & ADL training, EMR documentation, care coordination, and strong communication & time management skills.
