About the role
Provide occupational therapy services to patients with cognitive and ADL impairments in a long-term care setting. Evaluate patients, develop individualized treatment plans, provide evidence-based interventions, and collaborate with the interdisciplinary team to maximize patient independence and functional outcomes.
Responsibilities
- Perform comprehensive occupational therapy evaluations.
- Develop individualized treatment plans based on standardized assessments.
- Provide skilled interventions including ADL retraining, therapeutic activities, exercises, modalities, and adaptive equipment training.
- Monitor patient progress and modify treatment plans as needed.
- Educate patients, families, and caregivers on treatment plans and home strategies.
- Complete timely and accurate clinical documentation.
- Participate in care conferences and interdisciplinary meetings.
- Supervise Occupational Therapy Assistants, rehabilitation aides, and students as appropriate.
- Coordinate discharge planning and recommendations for DME and adaptive equipment.
Requirements
- Minimum 1 year of Occupational Therapy experience
- Long Term Care or Rehabilitation experience
- Active Oklahoma Occupational Therapy License
- Current skills checklist (within 1 year)
- Resume and reference required at submission
Preferred Qualifications
- Strong knowledge of ADL retraining and cognitive rehabilitation
- Experience in skilled nursing or post-acute rehabilitation
- New graduates with Level II Fieldwork (not observation only) will be considered
